- How many People will be attending the show with yourself
- Are you requiring a companion ticket if so, attach the relevant documentation for authorisation (ADP, Access Card, Assistance dog ID card, etc)
- Do you have any mobility/disability equipment (Walking Stick, Guide Dog, Crutches, Wheelchair)
- Attach Proof of Ticket Purchase
